What's Covered under the Allergy Care Plan? 

Lab Test Package (Prime Network)  

This package is made up of a number of predefined tests aimed at identifying and minimizing risks in addition to detecting illnesses early on. Policyholders are entitled to undergo a checkup each policy year for a pre-defined lab test package as per the package selected by them. These tests are available to only those listed under the policy. These tests can only be taken at Bajaj Finserv Health Prime partner hospitals and laboratories which are updated regularly. 

OPD / Doctor Consultation Benefit 

This benefit entitles policyholders to consult a doctor (specialty or general) that has a minimum degree of MBBS, BDS, BAMS, or BHMS, and the cover is provided for the same as per the amount specified under the plan. An invoice for this consultation should be provided such that reimbursement for the same can be processed. This reimbursement should clearly indicate the consultation fees, the doctor’s name and specialty, registration number, date of consultation, doctor stamp, and customer details for reimbursement. The benefit amount can be claimed in a single visit or can be spread across multiple visits to the doctor provided they fall within the benefit outlined in the policy.

Exclusions Under Lab Test Package 

  • This package cannot be availed of outside the Bajaj Finserv Health Prime network. 

  • The home collection facility is only available in select locations and in the event that it isn’t, policyholders need to physically visit the location. 

  • This package is only available to an adult mentioned in the policy and can’t be transferred to another member.

  • Carry forwards aren’t permitted even if that plan is renewed. 

  • Benefits aren’t redeemable against cash. 

Exclusions Under OPD/Doctor Consultation Benefit 

  • Coverage isn’t provided for any medical procedures a policyholder might undergo. 

  • Benefits may not be transferred to others unless they are covered under the policy.

  • Carryforwards are not permitted even if the plan is renewed,

  • The doctor consultation benefit can’t be added to another benefit. 

Exclusions to Availing Network Discounts 

  • Discounts cannot be clubbed with other benefits. 

  • Network discounts are only available within those specified under the Bajaj Finserv Health Prime network.

How to Make a Claim

Doctor Consultation Benefit/Lab and Radiology Benefit

1. Install the Bajaj Finserv Health app

2. Sign-up using the registered mobile number

3. Under the“Health Plans”, choose the purchased plan

4. Select ‘Doctor Consultation’ lab & radiology benefit’

5. Enter necessary details and upload the invoice

6. Share bank account details and upload a cancelled cheque

7. Submit the claim

8. The claim will be reimbursed within 48 working hours directly in the bank account 

 

  • Bajaj Finserv Health Website

1. Sign-up on the Bajaj Finserv Health website

2. Under the“Health Plans”, choose the Purchased plan

3. Select ‘Doctor Consultation’/‘lab & radiology benefit’

4. Enter necessary details and upload the invoice

5. Share bank account details and upload a cancelled cheque

6. Submit the claim

7. The claim will be reimbursed within 48 working hours directly into the bank account
