What's Covered under the Water Borne Disease Care

Here’s what’s covered under this insurance policy:

Lab Test Package (Prime Network)

• Consists of a predetermined set of tests that are aimed at identifying and minimising risk factors in addition to detecting certain illnesses and ailments at an early stage.

• Every policy year, the policyholder will get the option to undergo a checkup for a pre-defined lab test package as per your plan.

• Only adults subscribed to this policy are eligible to take the health care tests. The tests will only be available at Bajaj Finserv Health Prime partner labs and hospitals. 

 OPD/Doctor Consultation Benefit

• Doctor consultation benefit refers to and involves consultation with a doctor with either an MBBS, BDS, BAMS, or BHMS degree up to the benefit amount mentioned under your plan details. In case any medical speciality/specific doctor is offered under the plan,you can consult with a doctor/specialist of that specific speciality. In case speciality is not mentioned, you can consult any doctor or physician of your choice. Here are some of the benefits that you can avail:
• Consult with a doctor of your choice subject to a minimum degree of MBBS, BDS, BAMS, and BHMS (allopathic, dental, homoeopathic, and ayurvedic dental) as per the doctor speciality offered in the plan. Submit the invoice for reimbursement. The invoice should clearly state doctor name, doctor consultation fees, doctor registration number, doctor stamp, date of consultation, and customer details for reimbursement.  
• All members listed under the policy can avail this benefit.
• The benefit amount can be claimed in a single visit or can be spread out over multiple  doctor consultations to the extent of the value of benefits.

Network discounts

Discounts are offered on healthcare costs within the Bajaj Finserv Health Prime network. Here’s the list of discounts.

Note:

• Network Discounts are only applicable at the Bajaj Finserv Health Prime network. Please note that not all network discounts may be applicable at all Bajaj Finserv Health Prime networks.
• Network discounts can be utilised an unlimited number of times with no capping on the maximum discount that can be availed by the customer.

What’s not Covered in the Water Borne Disease Care?

Here’s what’s not covered under this plan:

Exclusions Under Lab Test Package

• The lab test package cannot be availed outside of the Bajaj Finserv Health Prime network.

• Home collection facilities will be available only at selected locations. For locations where home sample collection is not available, the customer will have to physically go and take the tests.

• Entire Lab test package will have to be utilised in one instance by any adult included under the policy. The benefit is non-transferable. 

• Unavailed benefits cannot be carried on, even on renewal of the policy after 1 year.

• Benefit(s) are not redeemable against cash.

Exclusions Under OPD/Doctor Consultation Benefit

• Reimbursements will not be made for any medical procedures that have been undertaken.

• Benefit(s) are not transferable to anyone else, not even to family members if they are not covered under the policy.

• Unavailed benefits cannot be carried on, even on renewal of the policy after 1 year.

• Benefit(s) are not redeemable against cash.

• Doctor consultation benefit cannot be clubbed with other benefits.

Exclusions To Availing Network Discounts

• Network Discounts cannot be clubbed with other benefits.
• Network discounts cannot be utilised outside of the Bajaj Finserv Health Prime network.

How to Make a Claim

Radiology and Lab Benefit/Doctor Consultation Benefit

The reimbursement claims for lab and radiology or doctor consultations benefits can be raised in the following ways:

Bajaj Finserv Health App

• Download the Bajaj Finserv Health app.

• Sign-in using your registered mobile number.

• Under ‘Health Plans’, select your purchased plan.

• Select ‘Doctor Consultation’ or ‘Lab & Radiology Benefit’, whichever is applicable.

• Enter the relevant details and upload your invoice.

• Share your bank account details and upload a cancelled cheque.

• Submit your claim.

• The claim will be reimbursed within 48 working hours and the reimbursed amount will be credited directly into your bank account.

Bajaj Finserv Health Website

• Sign-up/Sign-in on to the Bajaj Finserv Health website.

• Under my ‘Health Plans’, select your purchased plan.

• Select ‘Doctor Consultation’ or ‘Lab & Radiology Benefit’, whichever is applicable.

• Enter the relevant details and upload your invoice.

• Share your bank account details and upload a cancelled cheque.

• Submit your claim.

• The claim will be reimbursed within 48 working hours and the reimbursed amount will be credited directly into your bank account.

Emailing Customer Service

• Send an email to customercare@bajajfinservhealth.in.

• Attach a scanned copy of your invoice with all the details legible and visible.

• Mention details such as patient name, hospital/lab name, and bill amount.

• Share bank account details (bank name, account number, IFSC, primary account holder’s name) and also upload a cancelled cheque.

• The claim amount will be reimbursed within 48 hours and will be deposited into your bank account .

Tele-consultation Benefit

You can raise and avail of the benefit of teleconsultation or cashless claims, by following any of the methods listed below:

• Download the Bajaj Finserv Health app.

• Sign-up or sign-in using your registered mobile number.

• Under ‘My Health Plans’, select your purchased plan.

• Tap the ‘Teleconsultation Benefit’ option.

• Choose the doctor of your choice from the list of doctors.

• Select preferred date and time.

• Click on ‘Submit’.

• You will receive a link to join the call 15 mins before your consultation.

• The doctor will join the call during the scheduled day and time.

• Sign-up/sign-in using your registered mobile number

• Under my “Health Plans”, select your purchased plan

• Choose the ‘Teleconsultation Benefit’ option

• Select the doctor of your choice from the list of doctors

• Select your preferred date and time 

• Click on ‘Submit’

• You will receive a link to join the call 15 minutes before the consultation

• The doctor will join the call on the scheduled day and time

Lab Test Package (Prime Network Only)

• Visit the healthcare services offered by any hospital/lab, the lab/hospital should be a part of the Bajaj Finserv Health Prime Network.

• Share details regarding the benefit requested on the network to the billing executive. 

• You’ll receive an OTP, share it with the billing executive. 

• Upon sharing the OTP, you can get up to 10% discounts on select services at the hospital/network under the Bajaj Finserv Health Prime Network.
