1. Documents required as Proof of Identity

The documents you use as identity proof should contain your name and photograph. Here is the list of documents that you can provide as proof of your identity. 

• Voter Id

• PAN card

• Ration  card

• Passport

• Driving licence

• NREGS job card

• Any government-issued photo identity card 

• PSU-issued service photo identity card

• Photo identity card issued by an eminent and government-recognised educational institute

• Arms licence

• Credit card with photograph

• Debit card with photograph

• Bank passbook

• Kisan photo passbook 

• Pensioner photo card

• Freedom fighter photo card

• Marriage certificate with photograph

• CGHS (Central Government Health Insurance Scheme)/ECHS (Ex-servicemen Contributory Health Scheme) photo card

• Identity certificate with photograph, issued by a Gazetted officer/Tehsildar on a letterhead

• Address card with name and photograph, issued by the Department of Posts

• Certificate of identity with photograph, issued by an MLA/MLC/MP/Municipal Councillor, on their letterhead

• Disability Id card or handicapped medical certificate issued by the government of any state or union territory’s administration

• Certificate from an authority at a recognised educational institute, such as Superintendent, Marton, Warden, or the Head of the Institution

• Bhamashah Card

• SC, ST, or OBC certificate with photograph

• Certificate of identity with photograph issued by the Mukhiya or Village Panchayat Head

• RSBY (Rashtriya Swasthya Bima Yojana) card

• SSLC (Senior Secondary School Leaving Certificate) with a photograph

2. Documents required as proof of Date of Birth

Here is the list of documents that you may use as your date of birth proof.

• Birth certificate

• Bank passbook

• PAN card

• Educational marksheets issued by any government or board university

• Passport

• SSLC/STC (School Transfer Certificate) having name and date of birth

• Birth certificate issued by Group A Gazetted officer on letterhead

• Photo identity card having DOB details, issued by recognised educational institutions

• A certificate/identity card having photograph and DOB details duly signed and issued by a government authority

• Government photo identity card having DOB details 

• Central or state pension payment order

• CGSHA/SGHS photo identity card having DOB details

3. Documents required as  Proof of Relationship

This type of documentation is required if a minor applies for an Aadhaar card. In order to acquire it, he/she must produce a few documents proving his/her relationship with the family's head. Below are the details about the same.

• PDS (Public Distribution System) card

• Army canteen card

• Passport

• MNREGA job card

• Medical card issued by the state government, ECHS, CGHS, or ESIC

• Pension card

• Any family entitlement document issued by the central/state government

• Government-issued marriage certificate 

• Birth certificate issued by Registrar of Birth, Municipal Corporation, or any other local governmental notified body, such as Tahluk, Tehsil, etc.

• Bhamashah card

• Address card having name and photograph, issued by the Department of Posts

• Identity certificate containing a photograph issued on their letterhead by an MLA/MP/MLC/Gazetted Officer/Municipal Councillor

• Discharge slip issued by government hospital upon childbirth

• An identity certificate issued by the mukhiya/village panchayat having a photograph and relationship to the family’s head

4. Documents required as Proof of Address

Following is the list of documents that can be used as address proof.

• Voter Id card

• Utility bills (Gas, electricity, etc)

• Rent agreement 

• Post office account statement

• Credit card statement

• Tax receipt of the property

• Insurance policy documents

• Photo identity card issued by the government

• Photo and a letter with an address signed by the bank on letterhead

• NREGA job card

• Pensioner card

• Signed letter with address and photograph on the letterhead of the company issued by the registered office

• EGHS/CGHS card

• Address certificate comprising photo on a letterhead, issued by an MLA or MP or Tehsildar or Gazetted Officer

• Domicile certificate with photograph issued by the government

• Bhamashah card

• Vehicle registration certificate

• Disability Id card or handicapped medical certificate issued by governments and administrations of the respective state or union territory

• Passport of spouse

• Allotment letter issued by the government 

• A marriage certificate having address details 

• Certificate issued by authorised personnel of recognised shelter homes or orphanages
