Removing a Member

If a family member has moved out, passed away, or is now included under another ration card, you’ll need to remove their name from your card. Here is the process: 

• Step 1: Download or collect the ‘Name Deletion’ or ‘Member Removal’ form from the district / civil supplies website or office. 
• Step 2: Fill in the details asked, such as ration card number, name(s) of the person(s) to be removed, reason for removal (death, relocation, new card etc). 
• Step 3: Attach supporting documents as needed, like: 
  • Death certificate (if deceased)
  • Proof of another ration card (if moved to another card)
  • Proof of relocation/address change if moving outside the household/state 
• Step 4: Submit the completed form and documents to your Mamlatdar / ration‑card issuing office.
